Abstract: Introduction: Scintigraphy has long been the gold standard for indicating pyeloplasty for UPJO. The Pyeloplasty Prediction Score (PPS) was introduced as an alternative tool to indicate pyeloplasty. Ultrasound is widely available, painless, noninvasive, does not require radiation, and is a low-cost examination. This study evaluates the correspondence between PPS and patients who underwent pyeloplasty. Method: This was a retrospective study involving 120 patients who underwent pyeloplasty. After data collection, the researchers applied the PPS. Results & Discussion: The left kidney was the most commonly affected (68.3%), mainly in boys (67.5%), and 49.2% received an antenatal diagnosis. Intrinsic obstruction was found in 89.7% of the patients. Approximately 69.17% (n=83) had a preoperative PPS grater/equal 8. The affected kidney had a larger longitudinal measure than the contralateral. There was no difference related to sex, laterality, or etiology. The use of the ultrasound score would be sufficient for almost 70% of patients to indicate pyeloplasty without any other complementary exams. Conclusions: Based on this study, it is not possible to state that the PPS, without kidney scintigraphy, is effective in indicating pyeloplasty for every patient. This paper aims to explore transcendence as another fundamental human need within the framework of Max-Neef's Theory of Human Scale Development (THSD). While Max-Neef and other Latin American researchers, such as Carlos Mallman, have discussed various aspects of human needs, the explicit inclusion of transcendence as a fundamental human need in the framework HSD remains unresolved. However, Max-Neef's work provides foundations for reflecting on and potentially integrating transcendence into his original matrix of fundamental human needs. Given the current socioecological crises and global disasters such as wars, migrations and pandemic, there is an urgent necessity to reconsider our relationship with the planet and all forms of life on it. Transcendence, whether understood as transcending the ego or reconnecting with nature and life in its deepest sense, it is increasingly recognized both within and outside academia as a crucial dimension of the human experience. To support the argument for transcendence as a human need, this paper draws on insights coming from THSD, cognitive science, indigenous cosmologies, spirituality, and (eco)philosophy. Methodologically, the study employs a conceptual analysis with a hermeneutic approach to elucidate the nuanced understanding of transcendence in order to consider it as a universal human need.

Este artículo tiene como objetivo explorar la trascendencia como otra necesidad humana fundamental dentro del marco de la Teoría del Desarrollo a Escala Humana (TDEH) de Max-Neef. Aunque Max-Neef y otros investigadores latinoamericanos, como Carlos Mallman, han abordado diversos aspectos de las necesidades humanas, la inclusión explícita de la trascendencia como una necesidad humana fundamental en el marco de la TDEH sigue siendo un tema no resuelto. Sin embargo, el trabajo de Max-Neef proporciona bases para reflexionar sobre la posibilidad de integrar la trascendencia en su matriz original de necesidades humanas fundamentales. Ante las crisis socioecológicas actuales y los desastres globales, como guerras, migraciones y pandemias, surge una necesidad urgente de reconsiderar nuestra relación con el planeta y con todas las formas de vida en él. La trascendencia, ya sea entendida como la superación del ego o la reconexión con la naturaleza y la vida en su sentido más profundo, es cada vez más reconocida, tanto dentro como fuera del ámbito académico, como una dimensión crucial de la experiencia humana. Para respaldar el argumento de la trascendencia como una necesidad humana, este artículo se basa en aportes provenientes de la TDEH, la ciencia cognitiva, las cosmologías indígenas, la espiritualidad y la (eco)filosofía. Metodológicamente, el estudio emplea un análisis conceptual con un enfoque hermenéutico para esclarecer la comprensión matizada de la trascendencia y considerarla como una necesidad humana universal.

Background: Psoriasis is a chronic, non-contagious inflammatory skin disease with significant physical and quality-of-life impacts. In Brazil, its estimated incidence is 1.3%. Due to the complexity of the disease, effective management requires addressing multiple factors, with treatment adherence and persistence being critical challenges. Aim: To evaluate the correlation between psoriasis severity and treatment adherence and persistence among patients in Paraná, Brazil. Methods: This cross-sectional study included 133 psoriasis patients treated at the three primary specialized care centers in the state, between January 28, 2022, and December 9, 2022. Results: A negative correlation (−0.102, rs² = 1.04%) was observed between psoriasis severity and treatment adherence. Conversely, there was a positive correlation (0.2444, rs² = 5.97%) between psoriasis severity and treatment discontinuation history. Conclusion: Treatment interruptions were correlated with increased psoriasis severity, whereas higher adherence was associated with milder clinical manifestations. Relevance for patients: These findings underscore the critical role of consistent treatment adherence in managing psoriasis. Interruptions in treatment are linked to more severe forms of psoriasis, highlighting the detrimental effects of non-adherence. In contrast, patients who maintained high levels of adherence experienced less severe symptoms, emphasizing that consistent treatment is key to improved disease control. Strengthening patient-provider collaboration and adherence strategies can improve clinical outcomes and enhance quality of life.
