The effects of different exercise intensities on cognitive outcomes and brain-derived neurotrophic factor (BDNF) concentrations in adolescents with overweight/obesity are not yet fully elucidated. This study aimed: (a) to compare the prevalence of responders to cognitive function and BDNF concentration in adolescents with overweight/obesity participating in a 12-week intervention with high-intensity interval training (HIIT) or moderate-intensity continuous training (MICT), and (b) to analyze whether cardiorespiratory fitness (CRF) can explain differences in inter-individual variability between responders and non-responders. Adolescents with overweight/obesity were assigned to HIIT (n = 15), MICT (n = 14), or CG (n = 24). Anthropometrics, body composition, CRF, basal BDNF, and cognitive performance (Trial Making Test (TMT) and Stroop Test (ST)) were evaluated pre- and post-intervention. Training was performed on stationary bikes (3×/week for 12 weeks). The prevalence of responders in ST and TMT was higher in the HIIT group than in CG (93–62%, p = 0.029; 67–33%, p = 0.043), with no differences in MICT. No group differences were found for BDNF responders (HIIT:73%-MICT:71%-CG:46%, p = 0.263). ST and TMT responders had greater CRF improvements than non-responders. HIIT elicited a higher prevalence of cognitive responders than CG. CRF improvements may partially explain individual variability in responsiveness to cognitive outcomes.

Objective:
To assess the therapeutic efficacy of intravaginal dehydroepiandrosterone (DHEA) for vulvovaginal atrophy in postmenopausal women.
Evidence review:
A systematic literature search was performed in PubMed, Embase, and the Cochrane Library for studies published up to July 2025. Search terms included “DHEA,” “prasterone,” “Intrarosa,” and “dehydroepiandrosterone.” Randomized controlled trials (RCTs) evaluating intravaginal DHEA in postmenopausal women were included. Data extraction followed predefined inclusion and exclusion criteria. Risk of bias was assessed, and pooled analyses were conducted using random-effects models.
Findings:
Six RCTs representing five unique RCTs (n=1,611) involving postmenopausal women with VVA were included. Compared with placebo, intravaginal DHEA demonstrated significant improvements in two primary outcomes: vaginal dryness with a mean difference of −0.23 (95% CI, −0.35 to −0.11) and dyspareunia of −0.40 (95% CI: −0.66 to −0.15). No major safety concerns were reported, and adverse effects were mild and infrequent. The evidence consistently supported both statistical and clinical benefits of DHEA across trials, with low to moderate heterogeneity.
Conclusions and relevance:
Intravaginal DHEA significantly improves vulvovaginal symptoms, particularly vaginal dryness and dyspareunia, in postmenopausal women. These findings underscore its role as an effective and well-tolerated therapeutic option for the management of genitourinary syndrome of menopause, with potential to enhance quality of life....

Obesity is associated with comorbidities, including hypertension, which have been diagnosed in the pediatric population. The aim of this study was to verify the mediator role of physical activity in the relationship between anthropometric indicators and blood pressure (BP) in children with normal weight and children with overweight/obesity. The BeE-school project included 728 Portuguese children (376 boys, 352 girls), aged 6 to 10 years old, who were divided into two groups: children with normal weight and children with overweight/obesity, according to body mass index z-score (BMI-z). Body mass, height, waist circumference (WC), systolic BP (SBP) and diastolic BP (DBP) were assessed. Tri-ponderal mass index (TMI), waist-to-height ratio (WHtR) and mean BP (MBP) were calculated. Sedentary behavior and moderate-to-vigorous physical activity (MVPA) were determined by accelerometry. Mediation analyses were conducted considering predictors (WC, WHtR, TMI), mediators (MVPA on weekdays, weekend, total), outcomes (SBP, DBP, MBP). Children with overweight/obesity had higher mean values for BP measurements compared to their normal weight peers. In the correlations were not observed associations between anthropometric indicators and BP variables in children with normal weight, but MVPA on weekdays and MVPA total showed positive associations with SBP. However, children with overweight/obesity presented significant positive associations between all anthropometric indicators and BP variables. The present study provides evidence that physical activity functions as a competitive mediator in relationship between TMI and BP in children with overweight/obesity. The overweight simultaneously elevates BP directly while reducing protective physical activity engagement in Portuguese children.
